Procedure for the Issuance of an Emergency Passport

In the event of the loss or theft of a passport, and if the issuance of an emergency passport is required, it is important to follow the procedure below:
Steps to follow
- File a police report for the loss or theft of the passport.
- Contact the Belgian Embassy in Peru and provide as many details as possible regarding the loss or theft.
- Request an appointment through the Embassy for the issuance of the emergency passport at the Consulate.
- Attend the appointment with the following documentation:
- Signed emergency passport application form (document provided by the Embassy);
- Signed passport loss declaration (document provided by the Embassy);
- Police report for the loss or theft;
- Additional identity document (Belgian ID card or another passport);
- Photograph or old copy of the lost passport, if available;
- Passport-sized photographs;
- Flight itinerary.
Important
- The new emergency passport can only be issued if the complete documentation is provided.
- If any of the required documents are missing, the issuance cannot be carried out.
